The trigger was probably a malfunctioning level-crossing gate which prevented a green signal on the main line, an Indian Express report said. As a temporary solution, railway staff made changes to the 'location box' which is placed along the tracks and helps guide trains to their designated tracks.
On June 2, manual alteration of the location box gave a green signal to the oncoming Coromandel Express, but the point remained directed towards the loop line instead of the main line.
On the loop line, a stationary goods train was waiting, and was hit by the Coromandel Express.

The Railways said that it spent ₹13,523 crore from a non-lapsable Rashtriya Rail Sanraksha Kosh on track renewals in 2020-21, as per the Hindustan Times.
The Comptroller and Auditor General of India had estimated that ₹44,936 crore would be needed for this work, the report said.
The cause of the Odisha train accident is yet to be ascertained. At least 278 people died, while around 1,100 were injured in the June 2 incident.

Divisional Railway Manager of the Eastern Central Railways Rinkesh Roy told ANI that out of around 1,100 people injured in the incident, around 900 had been discharged after treatment. He added that around 200 people are still being treated in various hospitals in Odisha.
Regarding the death toll, Roy said that 278 people perished in the tragedy. He said that 177 bodies had been identified and handed over, or the process for it had started. Around 101 bodies were yet to be identified.

But this (State) visit is really about advancing what is now and what we hope will be a deeper, stronger partnership and friendship going forward," Kirby said in response to a question.
"There are innumerable reasons why India certainly matters, not just bilaterally between the two of our nations, but multilaterally on very many levels. And the president is looking forward very much to having Prime Minister Modi here to talk about all those issues and to advance and deepen that partnership and that friendship," Kirby said.

In the footage, some tourists are seen attacking each other and using paddles as weapons during a river rafting outing. While the cause of the fight is not known yet, the location makes it very dangerous.
Some persons are seen jumping into the river to escape the attack.

A Border Security Force (BSF) jawan was killed and two Assam Rifles personnel injured in an encounter with suspected Kuki insurgents in Manipur's Serou area early Tuesday, officials said.
The exchange of fire between the two sides took place in a school in Serou area of Sugnu in Kakching district, the officials said.
A BSF official said suspected Kuki miscreants resorted to indiscriminate and heavy firing targeted at BSF troops deployed at the Serou Practical High School around 4.15 am.
Constable Ranjit Yadav sustained a bullet injury during the gunfight and was evacuated to Kakching&rsquo;s Jivan Hospital where he was declared dead, the official said.
The two injured Assam Rifles personnel have been air evacuated to Mantripukhri and search operations are in progress, the Indian Army&rsquo;s SpearCorps, headquartered in Dimapur, added on Twitter.

"The accused has been booked under Indian Penal Code section 270 (malignant act likely to spread infection of disease dangerous to life)," he said.
According to police, accused Sameer (28) is from the Bareilly district of Uttar Pradesh.

The slogans of 'Khalistan Zindabad' were also raised by activists of Shiromani Akali Dal (Amritsar) led by MP Simranjit Singh Mann and his associate former MP Dhian Singh Mand at the Akal Takht. Mann was also present on the spot.
Radical Sikh outfit Dal Khalsa's activists were seen holding placards bearing portraits of slain militant leader Jarnail Singh Bhindranwale and raising pro-Khalistan slogans.
Hundreds of Sikh youths, led by the Dal Khalsa, were carrying Khalistani flags and photographs of damaged Akal Takht.
The entire marbled periphery of Golden Temple near Akal Takht, the supreme temporal seat of Sikhs, echoed with pro-Khalistan slogans.

He spoke about the issue of drug menace which has afflicted many youths.
He said Sikhs were never scared and panicked even after the incidents that happened in 1984.
"Rather all such incidents had made the Sikh community stronger and Sikhs would continue their struggle to get justice and would never be scared to stand with the truth," he said.
On this occasion, the Shiromani Gurdwara Parbandhak Committee (SGPC), the apex Sikh religious body, displayed the bullet-ridden holy 'saroop' (volume) of the Guru Granth Sahib.
The saroop, which was installed in the sanctum sanctorum at that time, was hit by a bullet during the army action in 1984.
