Odisha train accident: top official flagged 'serious signal system flaws' 3 months ago?

Serious concerns about the electronic interlocking system of the railways were raised by the chief operating manager of the South Western Railway Zone in February, according to a report by India Today. He had also stressed on the need for take safety measures.

The report comes as Union Railway Minister Ashwini Vaishnaw said that a 'change in electronic interlocking' was the reason behind the triple train crash in Odisha on June 2

India Today has reported that in a letter dated February 9 the principal chief operating manager of South Western Railway zone raised concerns of signal failure of an express train. He said that a head on collision with a goods train was averted due to the alertness of a loco pilot.

The letter mentions that the incident happened on February 8 and the train in question was Train No: 12649 Sampark Kranti Express.
