Japan's National Police agency on Thursday blamed loopholes in police protection citing laxity in planning, guarding for the July 8 assassination of its former PM Shinzo Abe.
Its national police chief Itaru NakamuraIts submitted his resignation to the National Public Safety Commission, as a way of taking responsilbity for the fatal shooting.
The resignation is expected to be approved at Friday's cabinet meeting, reported Japanese media.
The police chief said the country needs to have a new system to fundamentally re-examine guarding, and ensure that such incidents never happen in the future. 
In a 54-page investigative report, the National Police Agency concluded that the protection plan for Abe neglected potential danger coming from behind him and merely focused on risks during his movement from the site of his speech to his vehicle.
Inadequacies in the command system, communication among several key police officials, as well as their attention in areas behind Abe at the campaign site led to their lack of attention on the suspect's movement until it was too late.
The report noted that none of the officers assigned to immediate protection of Abe caught the suspect until he was already 7 meters (yards) behind him where he took out his homemade double-barrel gun, which resembled a camera with a long lens, to blast his first shot that narrowly missed Abe. Up to that moment, none of the officers was aware of the suspect's presence.

Shinzo Abe&rsquo;s term as Prime Minister of Japan not only deepened Tokyo&rsquo;s ties with India amid a common Chinese &lsquo;threat&rsquo; but also&nbsp;helped New Delhi enter the club of major democratic powers in the Indo-Pacific region.&nbsp;

Former Japanese Prime Minister Shinzo Abe died after being shot in the chest during an election campaign in the city of Nara in Japan on Friday leaving him seriously injured.
The 67-year-old was rushed to the hospital and was reportedly &ldquo;showing no vital signs&rdquo;. &nbsp;
As the news broke, wishes poured in from leaders from across the world. In India, Prime Minister Narendra Modi, Congress&rsquo; Rahul Gandhi, and Union Minister Jyotiraditya Scindia, among other leaders expressed their grief while stressing on the important role Abe played in strengthening India-Japan ties.&nbsp;
Abe as Japan's PM not only redefined Tokyo&rsquo;s ties with New Delhi amid a common Chinese &lsquo;threat&rsquo; but also helped New Delhi enter the club of major democratic powers in the Indo-Pacific region. He is often credited as the 'architect' of modern Indo-Japan ties.
Many firsts&nbsp;
In his first stint in 2006-07, then 52-year-old Abe visited India and addressed the Parliament. During his second stint, he visited India thrice&mdash;in 2014, 2015, and 2017&mdash; the most visits by any Japanese PM. &nbsp;
He was also the first Japanese PM to be Chief Guest at the Republic Day parade in 2014. The visit was credited to have helped the Indo-Japan ties reach the &ldquo;pinnacle&rdquo;. He was invited by the Congress-led UPA-II government that was ousted that year by the Bharatiya Janata Party (BJP)-led NDA government.&nbsp;
Deepening Indo-Japan, Indo-Pacific ties&nbsp; &nbsp; Although a &ldquo;global partnership&rdquo; and annual bilateral summits between the two countries were agreed upon in 2001 and 2005, respectively, Abe accelerated the process of deepening the ties.&nbsp;
During his first visit to India as PM in August 2007, Abe delivered the famous &ldquo;Confluence of the Two Seas&rdquo; speech&mdash;this idea about an arc of democracy stretching from the Pacific to the Indian Ocean now defines India-Japan ties.&nbsp;
During his second term, Abe helped build the relationship further.&nbsp;
Narendra Modi as PM chose Japan for his first bilateral visit in September 2014. The two leaders then agreed to upgrade the bilateral ties to &ldquo;Special Strategic and Global Partnership&rdquo; covering issues ranging from civilian nuclear energy to maritime security, bullet trains to quality infrastructure, Act East policy to Indo-Pacific strategy.&nbsp;
In 2017, the India-Japan Agreement for Cooperation in the Peaceful Uses of Nuclear Energy was signed. Abe&rsquo;s government played a key role in materialising the pact which faced resistance from anti-nuclear groups within Tokyo given India&rsquo;s exclusion from the 1970 Nuclear Non-Proliferation Treaty. The agreement helped India strike deals with US and French nuclear firms owned by or having stakes in Japanese firms.&nbsp;
Defence and infrastructure build up amid China 'threat'
Under Abe the two sides decided to have Foreign and Defence Ministers&rsquo; Meeting (2+2), a pact for transfer of defence equipment and technology was also signed in 2015.&nbsp;
In October 2017, as &lsquo;Chinese aggression&rsquo; grew in the Pacific, Indian Ocean, and India&rsquo;s borders in Doklam, Abe Japan pushed the idea of reviving the Quad--a Japan, US, India, and Australia grouping. &nbsp; During India-China border stand-offs over the years, including the one in Doklam in 2017, Abe&rsquo;s Japan firmly stood with India while asking China to change status quo.&nbsp;
Under Abe, India and Japan formed the Act East Forum that promotes projects in the Northeast and planned joint projects in Maldives and Sri Lanka in a bid to counter China's growing influence.&nbsp;

Japan's governing party and its coalition partner scored a major victory in a parliamentary election Sunday imbued with meaning after the assassination of former Prime Minister Shinzo Abe amid uncertainty about how his loss may affect party unity.
The Liberal Democratic Party and its junior coalition partner Komeito raised their combined share in the 248-seat chamber to 146 — far beyond the majority — in the elections for half of the seats in the less powerful upper house.
With the boost, Prime Minister Fumio Kishida stands to rule without interruption until a scheduled election in 2025.
That would allow Kishida to work on long-term policies such as national security, his signature but still vague "new capitalism" economic policy, and his party's long-cherished goal to amend the U.S.-drafted postwar pacifist constitution.
A charter change proposal is now a possibility. With the help of two opposition parties supportive of a charter change, the governing bloc now has two-thirds majority in the chamber needed to propose an amendment, making it a realistic possibility. The governing bloc already has secured support in the other chamber.
Kishida welcomed the major win but wasn't smiling, given the loss of Abe and the hard task of unifying his party without him. In media interviews late Sunday, Kishida repeated: "Party unity is more important than anything else." He said responses to COVID-19, Russia's invasion of Ukraine and rising prices will be his priorities. He said he will also steadily push for reinforcing Japan's national security as well a constitutional amendment.
Kishida and senior party lawmakers observed a moment of silence for Abe at the party election headquarters before placing on the whiteboard victory ribbons next to the names of candidates who secured their seats.
Abe, 67, was shot while giving a campaign speech in the western city of Nara on Friday and died of massive blood loss. He was Japan's longest-serving political leader over two terms in office, and though he stepped down in 2020 was deeply influential in the LDP while heading its largest faction, Seiwakai.
"This could be a turning point" for the LDP over its divisive policies on gender equality, same-sex marriages and other issues that Abe-backed ultra-conservatives with paternalistic family values had resisted, said Mitsuru Fukuda, a crisis management professor at Nihon University.
Japan's current diplomatic and security stance is unlikely to be swayed because fundamental changes had already been made by Abe. His ultra-nationalist views and pragmatic policies made him a divisive figure to many, including in the Koreas and China.
Following the assassination, Sunday's vote took on new meaning, with all of Japan's political leaders emphasizing the importance of free speech and defending democracy against acts of violence.
Abe's killing may have resulted in sympathy votes. Turnout on Sunday was around 52%, up about 3 points from the previous 48.8% in 2019.
"It was extremely meaningful that we carried out the election," Kishida said Sunday. "Our endeavor to protect democracy continues." On the final day of campaigning Saturday, party leaders avoided fist-bumps and other friendly gestures in close contact with the public — a sign of tightened security following Abe's assassination during a campaign rally.
Abe's body has been returned to his home in Tokyo's upscale Shibuya, where many mourners, including Kishida and top party officials, paid tribute. His wake and funeral are expected in coming days.
On Sunday, the suspect accused of his murder was transferred to a local prosecutors' office for further investigation, and a top regional police official acknowledged possible security lapses allowed the gunman to get close to Abe and fire his homemade gun at him.
The suspect, Tetsuya Yamagami, told investigators he acted because of Abe's rumored connection to an organization that he resented, police said, but had no problem with the former leader's political views. The man hated a religious group that his mother was obsessed about and that bankrupted a family business, according to media reports, including some that identified the group as the Unification Church.
Nara prefectural police chief Tomoaki Onizuka said Saturday that problems with security were undeniable, that he took the shooting seriously and will review the security procedures.
Japan is known for its strict gun laws. With a population of 125 million, it had only 21 gun-related criminal cases in 2020, according to the latest government crime paper. Experts say, however, some recent attacks involved use of consumer items such as gasoline, suggesting increased risks for ordinary people to be embroiled in mass attacks.
