A troubling new study reveals that drug use among Meghalaya's youth begins alarmingly early, with some starting as young as 10 years old, significantly earlier than the average initiation age of 22 in neighboring Manipur and Nagaland.
Published in the April-June 2025 edition of the Indian Journal of Public Health, the qualitative research was conducted by the Indian Institute of Public Health in Shillong and the Meghalaya AIDS Control Society.
Researchers surveyed 128 participants who were enrolled in opioid substitution therapy programs across the regions of East Khasi Hills, West Jaintia Hills, and East Jaintia Hills, in addition to interviewing 17 service providers.
The study found the mean age of drug initiation to be 19, with more than a third of respondents beginning as minors and a quarter still in school at the time.
According to the study, an overwhelming 91 percent of the drug users are male. Notably, 80 percent are employed in the unorganised sector, and over half have received less than 10 years of formal education.
Key factors driving initial drug use include curiosity, family problems, peer pressure, and the need to relieve stress, noted the study. Heroin was identified as the most commonly injected drug, with daily usage costs ranging from Rs 500 to Rs 2,500.
Access to drugs was described as "easy," achieved through marketplaces and mobile transactions. Once the initial phase of peer sharing subsided, many users began borrowing or stealing money, the study found.
Researchers warned that early initiation heightens the risk of long-term addiction, linking this trend to a breakdown in traditional community support systems. While polydrug use was rare, many users transitioned from pills or marijuana to heroin over time.
Furthermore, potent heroin variants locally known as 'black tiger' and 'double tiger' were reported to be especially addictive. The study urgently calls for outreach programs in educational institutions and community spaces to prevent early drug initiation and tackle the increasing usage trend.

Punjab Health Minister Dr. Balbir Singh today urged Union Health and Family Welfare Minister Mr. J. P. Nadda to enforce stricter regulations on e-pharmacies, specifically requesting that they be prohibited from selling psychotropic drugs without a valid prescription. This appeal aligns with ongoing efforts in Punjab to combat drug abuse and aligns with existing national guidelines under the Drugs and Cosmetics Act, 1940, and the Narcotic Drugs and Psychotropic Substances Act, 1985, which regulate the sale of such substances. Dr. Singh&rsquo;s request reflects concerns over the potential misuse of online platforms, emphasizing the need for tighter controls to ensure these drugs are dispensed only under medical supervision, supporting Punjab&rsquo;s broader campaign to curb the drug menace in the state. Punjab Cabinet Minister revealed that the Union Health Minister agreed to this.
On the issue of Pending &nbsp;dues of State Health Agency under Ayushman Insurance Scheme, the Ministry of Health, Government of India has agreed to release about Rs. 54 Crores to State of Punjab for the successful operation of the scheme.
Mr. Nadda assured Dr. Singh that about Rs. 50 Crores will be released after the completion of few formalities in subsequent weeks/months.
In a significant step towards combating drug misuse in Punjab, the Union Health Minister has agreed to a proposal from the Punjab Health Minister to include Pregabalin and Tapentadol in Schedule H1 of the Drugs Rules, 1945, Dr. Singh further revealed.
He also informed the Union Health minister that, Pregabalin, widely used for neuropathic pain and epilepsy, and Tapentadol, a potent opioid analgesic, have been identified as substances of concern due to their increasing misuse in Punjab and beyond. The Punjab government has reported significant seizures of these drugs, including Pregabalin formulations worth over ₹5 crore in 2024, highlighting the urgency of stricter regulations.
By classifying these drugs under Schedule H1, their sale will be restricted to prescriptions from registered medical practitioners, with mandatory record-keeping by pharmacies to prevent over-the-counter access and misuse.
Punjab Cabinet Minister said that the Union Health Minister commended Punjab&rsquo;s proactive stance in addressing the drug crisis and reiterated the central government&rsquo;s support for state-led initiatives to safeguard public health. This amendment to the Drugs Rules is expected to be formally notified soon, following procedural formalities.
The Ministry urged all stakeholders, including healthcare providers and pharmacies, to comply with the updated regulations once enforced.

The Supreme Court has asked the Punjab government to issue a circular stating that local police will be held accountable for illegal liquor manufacturing units (bhattis) operating under their vigil.
The directions were issued by a bench of Justices MR Shah and CT Ravikumar on a petition challenging an order of the Punjab and Haryana High Court, which had refused to transfer the probe into the cases of illegal liquor manufacturing to the Central Bureau of Investigation (CBI). According to the petitioner, there is a bhatti in every mohalla (locality) in Punjab.
The Court remarked that this was unfortunate and said that the police have to make every effort to save the country, as it is very easy to ruin the youth, especially in border states like Punjab. "This is unfortunate. Punjab is a border State. If somebody wants to finish the country, they will start with the borders. They (the police) have to take every effort to save the country. It is very easy to ruin the youth," the Court said.
The Court, therefore, directed the State to file a reply mentioning steps taken by it to fight domestic, commercial production of illicit liquor and how seized funds will be used for awareness campaigns or increasing manpower.
It further suggested that the State come out with a circular that if an illegal bhatti is found, the concerned local police will be held responsible for not being vigilant. The matter will be heard further on December 12, 2022.

Bollywood actor Shraddha Kapoor's brother Siddhanth Kapoor was released on bail late on Monday after he was arrested for alleged consumption of drugs. 
The other four people arrested have also been released on bail, said police.
Siddhanth Kapoor and the four others will have to further appear before the police as and when called, said Bheema Shankar Gulled, Deputy Commissioner of Police for East Bengaluru.
Siddhanth Kapoor was detained by Bengaluru Police earlier in the day for allegedly consuming drugs at a party in the city on Sunday night.
Guled said, "Siddhanth Kapoor's medical shows he had consumed drugs. We have already arrested him and are following the procedure. We will be sending him to judicial custody."
Acting on a tip-off, police raided a hotel on MG Road in the city on Sunday night where a party was hosted.
"Last night we got info that a party is going on and they have consumed drugs. We raided and detained 35 people. We didn't find any drugs on their person but found MDMA and ganja disposed of nearby. We will investigate CCTV to check who disposed it of," added DCP Guled.
Samples of people suspected of having consumed drugs were sent for medical tests and Siddhanth's sample tested positive. Police said it was unclear whether they had consumed drugs at the hotel or had come to the party after taking drugs outside.
"The hotel has been given notice, we have asked specific questions, they will have to answer. Bengaluru city police have declared war on drugs and this was part of it. Earlier in the month, we had conducted a raid where 34 people tested positive for drugs," added the senior police officer.
